Camp Kid's Stop
Enrollment Instructions
Review all forms and paperwork carefully before completing. Please type or print all necessary information. Forms must be complete and legibly filled out for your registration to be processed. Processing may be delayed if we are unable to clearly read and understand the information requested.
All children attending CAMP KID’S STOP must have a current health assessment on file at the time of initial attendance. This health assessment is carefully reviewed by our licensing agency and must contain complete and current immunization and health screening information required by the American Academy of Pediatrics.
Return completed forms along with the non-refundable registration fee (payable to MN Leisure Services) to:
